To fill out the forms in Adobe Reader please do the folowing:
- Download the most up to date version of Adobe Reader by clicking here
- Save the file to your computer first before opening it. Do not open it in your browser and fill it out. You will lose all your data.
- To download the form either left click on the link and click save (not open) or right click on the link and select 'save link as' or something similar to that.

Step 2
Fill in the registration form using Acrobat Reader version 8 or 9. You may save the file with your changes and open it later to make changes.
Step 3
Submit the form in one of these ways.
- Email the file as an attachment to atlanta_registration@seypw.org (preferred).
- Print the file and fax it to (404) 751-2772.
- Print the file and mail it to the Church in Atlanta to arrive on or before Fri, Feb. 12th.
Step 4
Mail one check, payable to “The Church in Atlanta” to:

To avoid the late payment fee please have the check post marked by Fri., Feb. 12th.
